October 2024

Disclosure and Raising a Concern

In October 2024, relevant sections of this chapter were updated to include information about concerns relating solely to pressure ulcers. Also, information was added about the requirement that local authorities should notify Ofsted of the death of any care leaver under the age of 25, where it is aware of their care leaver status was added in Section 6, If the Adult has Died.

Allegations against Service Providers (including Organisational Abuse)

Section 6, Concerns about Pressure Ulcers and Section 7, Concerns about the use of Restraint in Mental Health Units were added in October 2024.

Domestic Abuse (including MARAC and MATAC)

In October 2024, information about Stalking Protection Orders was added to Section 3, Specialist Domestic Abuse Services and Support. Information about the timeframe for introduction of Domestic Abuse Protection Orders and Domestic Abuse Protection Notices was also updated.

LeDeR Reviews

In October 2024, a link to the LeDeR online referral was added.

In October 2024, this chapter was replaced with a revised and updated version.

Allegations against People in a Position of Trust

Section 1, What is a Position of Trust was updated in April 2024 to include people that provide regular coaching, teaching or instruction in sport or religion.

Prevent – Safeguarding Adults against Radicalisation and Violent Extremism

In April 2024, this chapter was replaced with a revised and updated version.

Mate Crime

In April 2024, this chapter was renamed ‘Mate Crime’. It was fully reviewed and refreshed. Information about cuckooing (also known as forced home invasion) was relocated to a new chapter.

County Lines

In April 2024, this chapter was fully reviewed and refreshed in line with updated Home Office guidance ‘Criminal exploitation of children and vulnerable adults: county lines’.

Allegations against Service Providers (including Organisational Abuse)

Section 6, Making Enquiries was updated in April 2024 to provide greater clarity about the responsibility of making enquiries.

Safeguarding Adult Reviews (SAR)

In April 2024, a new Section 9, Safeguarding Adults Reviews and Rapid Time (SARiRT) was added.

Progressing a Concern to a Section 42 Enquiry

In April 2024, this chapter was renamed from 'Responding to a Concern' and locally reviewed. Additionally, a new Section 7, If the Adult has Died was added containing information previously located in the chapter Disclosure and Raising a Concern.

Deciding Whether to Submit an Adult Safeguarding Concern

In April 2024, this new local chapter was added to the Safeguarding Adults Core Procedures section.

Disclosure and Raising a Concern

In April 2024, this chapter was reviewed locally. Additionally, Section 6, If the Adult has Died was updated to clarify that a concern should still be raised if the death may have been as a result of abuse or neglect.
